Dog Adoption Application

This information helps PAWS find permanent, responsible, loving homes for the animals and allows us to better assist you in finding your perfect pet. Prior to approval, PAWS will run a background check.

If you don't own your home, we must contact homeowner or rental agency.

Your veterinarian may be contacted to confirm your pet's current rabies vaccination. Please list multiple vets, if needed.

Are you planning to move in the next 6 months?
Does anyone in your household have animal allergies?
Pets making the transition from a shelter to a new home need time to adjust to a new family and may require house training and behavior training. Are you willing to provide any needed training?
Will this be your first dog?
Pets are an investment of your time and money. Can you afford to provide medical care, grooming, proper diet, shelter, and exercise for your new dog (approx: $1,400 yearly)?
Are you able to make a long-term commitment to care for your pet for its entire life span, which could be 15+ years or more?
PAWS does not adopt out dogs to be guard, outdoor, bait/fighting or hunting dogs. Do you understand and agree to these terms?
Is your yard fenced in?

We reserve the right to refuse adoption to any applicant for any reason. This questionnaire becomes part of our contract. Do you understand and agree that everything on this application is true to the best of your knowledge? Sign below.
