This information helps PAWS find permanent, responsible, loving homes for the animals, and allows us to better assist you in finding your perfect pet. Prior to approval, PAWS will run a background check.

If you don't own your home, we must contact homeowner or rental agency.

This pet is for your child(ren) to teach responsibility?
Are you familiar with this animal’s husbandry?
Have you ever had to give away/rehome pets previously?

I have answered the screening questions truthfully to the best of my ability. I understand that any misrepresentation of this information is grounds for adoption refusal.

I understand that my newly adopted pet may be spayed/neutered upon adoption and prior to leaving the PAWS property, and if it is not, I will have it sterilized at a veterinarian facility of my choosing, at my own cost, and I WILL NOT breed this animal whatsoever.
